Fever, aches from Pfizer, Moderna jabs aren’t dangerous but may be intense for some

This summer, Luke Hutchison, a Massachusetts Institute of Technology–educated computational biologist, volunteered for a trial of Moderna’s COVID-19 vaccine. After he got the second injection, his arm immediately swelled up to the size of a “goose egg,” Hutchison says. He can’t be sure he got the vaccine and not a placebo, but within a few hours, the healthy then-43-year-old was beset by bone and muscle aches and a 38.9°C fever that felt, he says, “unbearable.” “I started shaking. I had cold and hot rushes,” he says. “I was sitting by the phone all night long thinking: ‘Should I call 911?’”

He says the public should be better prepared than he was, because a subset of people may face intense, if transient, side effects, called reactogenicity, from Moderna’s vaccine. Some health experts agree.

Both vaccines consist of a snippet of genetic code directing production of the coronavirus’ spike protein, delivered in a tiny fat bubble called a lipid nanoparticle. Some suspect the immune system’s response to that delivery vehicle is causing the short-term side effects.

Fewer than 2% of recipients of the Pfizer and Moderna vaccines developed severe fevers of 39°C to 40°C. But if the companies win regulatory approvals, they’re aiming to supply vaccine to 35 million people globally by the end of December. If 2% experienced severe fever, that would be 700,000 people.

That’s a higher rate of severe reactions than people may be accustomed to. “This is higher reactogenicity than is ordinarily seen with most flu vaccines, even the high-dose ones,” says Arnold Monto, an epidemiologist at the University of Michigan School of Public Health.

“We suspect the lipid nanoparticle causes the reactogenicity, because lipid nanoparticles without mRNA in them do the same thing in animals,” Weissman says. “We see production, in the muscle, of inflammatory mediators that cause pain, [redness], swelling, fever, flulike symptoms, etc.”

Twelve hours after receiving his second injection in May, Haydon got chills as well as “headache, muscle ache, fatigue, nausea,” and had a fever of 39.6°C. He went to urgent care, and later vomited and fainted before the symptoms receded, roughly 24 hours after they started, he says.

Longer term side effects of mRNA vaccines remain theoretical. They include the possibility that people with lupus, whose disease is driven by antibodies against their own genetic code, could experience flare-ups because of the revved up immune response induced by the vaccines, says Sarfaraz Hasni, director of lupus clinical research at the National Institute of Arthritis and Musculoskeletal and Skin Diseases.

But there’s no evidence currently that mRNA vaccines cause autoimmune disease or make it worse, says Betty Diamond, an immunologist and rheumatologist at the Feinstein Institutes for Medical Research of Northwell Health. “At the moment there’s every reason to suggest that people with autoimmune diseases ought to get either of these vaccines when they get rolled out.”

Kanger International Berhad is closer to its aim of becoming the world’s largest producer of bamboo products when its new bamboo processing plant and R&D centre in Jingzhou, China goes into full production in the fourth quarter of 2020.

The new factory and R&D centre is part of a public private arrangement with the local Jingzhou government on a 70:30 basis, and was built at a total investment cost of RM121 million.

Snap polls will power KLCI to 1,800 in 2021, says asset management firm

Inter-Pacific Asset Management Sdn Bhd CEO Nazri Khan (DDNK) said investors have been anticipating a snap election as early as January next year due to optimism over the introduction of the Covid-19 vaccine, along with expectations of the removal of the conditional movement control order (CMCO).

“Many investors believe that the Covid-19 vaccine is likely to be available in the country around March 2021.

“Many of them expect once the pandemic is stabilised and the CMCO removed, the snap election can happen as soon as January 2021,” he said.

“This (snap election) will be one of the biggest catalysts for the market’s BULLISHNESS next year,” he told Bernama.

On the external front, DDNK said US President-elect Joe Biden’s victory is expected to have a positive impact on global equity markets, especially that of China, as well as other Asian and emerging markets.

“Biden seems to be less confrontational, less US-centric and more market-friendly. Many believe he will welcome more free trade and a liberal global trade system moving forward.

Some are also being donated via a subsidized program, COVAX via the WHO. Countries that have ordered Chinese manufactured vaccines so far include Indonesia, Sao Paulo province in Brazil (despite disruptions from the central government), Mexico, Malaysia (donation), Turkey and the Philippines. The scope of countries to receive donations from Beijing is also likely to be ramped up as China intensifies its multilateral cooperation to fight the COVID-19.

Once Pfizer vaccine has been thawed, it requires dilution prior to injection whereas Moderna vaccine does not require dilution.

Pfizer vaccine : Once thawed, undliluted vial can be stored in a refrigerator for only 5 days and the diluted vial can only be kept for 6 hours.

Moderna vaccine : Once thawed, vial can be stored in a refrigerator for up to 30 days and at room temperature for up to 12 hours.

Right now, Pfizer says its vaccine needs to be kept at minus 70 degrees Celsius and can last in a specialty freezer for up to six months. The specialty shippers can hold up to five "pizza box" trays of vials and be refreshed with dry ice every five days for up to 15 days to keep the vaccine at the right frozen temperature.

Even that presents challenges, though — a Pfizer scientist told a CDC advisory council in August that it's not supposed to be opened more than twice a day and needs to be closed within one minute of opening. Once it's thawed, the vaccine can be refrigerated for five days.

Moderna says its vaccine candidate is stable at regular freezer temperature — minus 20 degrees Celsius — for up to six months, and after thawing it can last in the refrigerator for 30 days. It can also be kept at room temperature for up to 12 hours. This, explains Kristensen, is useful for health care workers in the field, "because now the vaccine doesn't need to go in and out of the refrigerator each time it's administered."

@eyehunter Sinopharm vaccine storage temperature is the standard storage temperature for most other approved vaccines in the world ie 2-8C. This is in contrast to Oral polio virus, Pfizer's mRNA vaccine and Moderna's mRNA vaccine which requires storage at subzero temperatures.

Sinopharm chose the United Arab Emirates to conduct their phase III clinical trial of the Covid Vaccine because there are around 200 different nationalities residing there and UAE has a focus on medical research and fighting the pandemic. This is great because that would mean that the results of the phase III clinical trial will be applicable to all races. As we know, different races have different genetic traits and makeup which can potentially make a vaccine more effective and safe in one race but less effective and safe in another race.

This is precisely the reason why a bridge clinical trial is required by the China National Medical Products Administration (NMPA) for pharmaceutical products (including vaccines) which are approved abroad but do not have data to show that ethnic differences can affect their efficacy and safety. The commencement of the clinical trial for Pfizer's mRNA vaccine in China is a good case in point.

It is great to know that the World Health Organization (WHO) is overseeing the phase III clinical trials of Sinopharm vaccine. This will ensure that once the phase III trials are complete and the vaccine obtains approval from National Medical Products Administration (NMPA), China, the vaccine will also stand a high chance of successfully obtaining prequalification from WHO. Obtaining WHO prequalification of vaccines is a great achievement as it would mean that the vaccine meets internationally recognized standards of quality, safety and efficacy.

@purple_rain No no, that is not going to happen anytime soon. Malaysia will have to import vaccines as a finished product first because vaccine fill and finish will take time to train personnel and for the technology transfer. I foresee vaccine fill and finish might only commence 3rd or 4th quarter of 2021. So far no local manufacturers have ever conducted fill and finish of vaccines in Malaysia. Currently only Pharmaniaga and Duopharma has the fill and finish facility for vaccine certified by NPRA but they have not yet commenced any fill and finish activities for any vaccines yet. As for Solution group, their fill and finish facility for vaccines is still undergoing inspection by NPRA (not yet approved) and they are targeting to commence vaccine F&F facility by 2Q 2021 (Refer to this link https://www.theedgemarkets.com/article/solution-group-targets-commence-vaccine-ff-facility-2q-2021)
