Hydroxychloroquine: US revokes emergency approval of malaria drug for Covid-19
Food and Drug Administration says drug is unlikely to work against coronavirus and notes heart risks

US regulators revoked the emergency authorization for malaria drugs championed by Donald Trump for treating Covid-19, amid growing evidence they don’t work and could cause serious side effects.

The Food and Drug Administration (FDA) said on Monday the drugs hydroxychloroquine and chloroquine were unlikely to be effective in treating the coronavirus. Citing reports of heart complications, the agency said the drugs’ unproven benefits “do not outweigh the known and potential risks”.

Trump criticized the FDA decision. “I took it and I felt good about taking it. I don’t know if it had an impact, but it certainly didn’t hurt me,” Trump said

Trump said there had been “great reports” out of France, Spain and other places, without offering any evidence or further explanation. France is one of the countries that has already stopped using the drug for Covid-19 patients.

In a separate announcement, the FDA also warned doctors against prescribing the drugs in combination with remdesivir, the lone drug currently shown to help patients with Covid-19. The agency said the anti-malaria drugs could reduce the effectiveness of remdesivir, which the FDA cleared for emergency use in May.

Hydroxychloroquine and chloroquine are frequently prescribed for lupus and rheumatoid arthritis.

The FDA granted emergency use for the anti-malaria drugs in late March at the same time the US government accepted 30m doses of hydroxychloroquine and chloroquine that had been donated by two foreign drug manufacturers. Millions of those doses were shipped to US hospitals to treat patients who weren’t enrolled in clinical trials.

Meanwhile, no large, rigorous studies have found the drugs safe or effective for preventing or treating Covid-19. And a string of recent studies made clear they could do more harm than good.

The drugs can cause heart rhythm problems, severely low blood pressure and muscle or nerve damage. The FDA reported on Monday that it had received nearly 390 reports of complications with the drugs, including more than 100 involving serious heart problems. Such reports represent an incomplete snapshot of complications with the drugs because many side effects go unreported.

The FDA’s move means that shipments of the drugs obtained by the federal government will no longer be distributed to state and local health authorities for use against the coronavirus. The decades-old drugs are still available for alternative FDA-approved uses.

Dr Steven Nissen, a Cleveland Clinic researcher who has been a frequent FDA adviser, agreed with the decision and said he would not have granted emergency access in the first place.

“There has never been any high-quality evidence suggesting that hydroxychloroquine is effective” for treating or preventing coronavirus infection, he said, but there was evidence of serious side effects.

In March, Trump said hydroxychloroquine used in combination with the antibiotic azithromycin had “a real chance to be one of the biggest game changers in the history of medicine”, with little evidence to back up that claim.

He later said he took the drugs preventively after two people who worked at the White House were diagnosed with Covid-19, and he urged others to try it.

After Trump’s repeated promotions, prescriptions for hydroxychloroquine soared, contributing to shortages.

The only remaining drug with FDA authorization against Covid-19 is remdesivir, an intravenous medication from Gilead Sciences that has been shown to help severely ill, hospitalized patients recover faster.

This is a drug to reduce the viral load.

Two other areas of concern are replication of the virus and the immune system's noted over reaction to the perceived aggressiveness -- Covid-19 -- of the novel Corona virus in the epithelial cells and lymph nodes.

The over reaction may cause cold, cough, fever, pneumonia, low oxygen, high CO2 levels, breathing difficulties, blood clots, kidney problems, complications and systemic distress.

It was report early during Covid 19 crisis that Malaysian scientists were working on a vaccine! Not sure if it was a joke or not but they did report it, quoting some government agency (not sure if it was a NGO or MOH)?

The use of hydroxychloroquine (HCL - not Hyrogen Sulphate) has attracted some serious discussion and it makes sense that the DG had decided to pull out its use. We do appreciate his interest and concern in saving lives and he his effort has in the various measures initiated and implemented by our MKN has returned the country to near normalcy. Notwithstanding, the use of HCL has raised questions.

This health DG was adamant that Plaquenil was the reason why Malaysia had so few deaths compared to the western countries. Now he is making an about turn saying evidence show no benefits of hydroxychloroquine. It’s ok to not know something but to try defend something when the evidence was already to the contrary and even to the extend of making a conclusion that it reduced mortality is downright irresponsible. If the health DG could lie about treatment, what guarantee is there that he will not be lying about other data.

Would like to know when the govt dental departments are going to open...dental doctors are giving the excuse..we dont do fillings...dentures...and so on...only emergency cases are treated..in spite of all the bonuses and allowances given to them during covid (they deserve it by the way)...however its time to start their duties as there is a backlog..and we senior citizens need to do our dentures as we cant eat properly..come on ..give us a date...most of the dental drs and nurses are sitiing there playing with their phones and gossipping....come on health minister ...do something.

The European Union told China on Monday to make good on a promise to open up its economy and warned of “very negative consequences” if Beijing goes ahead with a new security law on Hong Kong that the West says will curtail basic rights.

Speaking after video calls with Chinese Premier Li Keqiang and President Xi Jinping, the EU’s chief executive and chairman said they had repeated accusations that Beijing has spread disinformation about the coronavirus.

“The relationship between the EU and China is simultaneously one of the most strategically important and one of the most challenging that we have,” European Commission President Ursula von der Leyen told a news conference.

European Council President Charles Michel said China was not reciprocating the welcome that Chinese companies receive in Europe.

Calling China a partner and a rival, von der Leyen said Beijing had not followed up on a 2019 deal to allow greater access for European companies in China or drop rules requiring investors to share their know-how in Chinese joint ventures.

When asked about von der Leyen’s comments on the 2019 deal on Tuesday, Wang Lutong, the head of the Chinese foreign ministry’s Europe office, said that tangible progress has been made on areas like green financing and government procurement, and that patience was needed.

Even before the coronavirus pandemic worsened Sino-European ties, the EU found itself caught between China and the United States, needing both and reluctant to alienate either.

Li expressed optimism about the relationship, according to Chinese state media, saying China and the EU are more partners than competitors.

But the EU wants to see progress on an investment agreement under negotiation since 2014.

EU officials say they want to see movement in areas such as autos, biotech and micro-electronics and see Beijing limit subsidies for state-run companies.

Germany has postponed an EU leaders’ summit with Xi in September, citing the coronavirus, though diplomats said it was in part because of the impasse in investment negotiations.

Michel and von der Leyen said they told Li and Xi of their concerns over China’s security law for Hong Kong, which democracy activists, diplomats and some businesses say will jeopardise its semi-autonomous status and role as a global financial hub.

China’s parliament reacted angrily on Saturday to a resolution by the EU assembly protesting against the security law.

“We also conveyed that China risks very negative consequences if it goes forward with imposing this law,” von der Leyen said, without giving details.

“The European Union is in touch with our G7 partners on this topic and we’ve made our position very clear to the Chinese leadership today and urged them to reconsider.”
